Introduction and purpose of study
You are invited to participate in a survey regarding the cleanliness and maintenance of buildings at the University of Toronto. The purpose of this study is to examine how users' daily routines and building usage patterns influence their awareness of and satisfaction with the cleanliness of university facilities?

Procedures
If you agree to participate, you will be asked to complete an online survey. The survey consists of 13 questions and is expected to take approximately 5 minutes to complete. You will be asked about your experiences with specific buildings and your satisfaction levels with facility maintenance.

Voluntary participation
Your participation in this research is entirely voluntary. You may choose to skip any questions you do not wish to answer, and you may stop the survey at any time by closing your browser window. If you choose to withdraw before submitting, your responses will not be recorded. Due to the anonymous nature of this survey, once you hit submit we will be unable to remove your specific data as it will not be linked to your identity.

Risks and benefits
There are no known risks to participating in this study. While there is no direct personal benefit to you, your feedback will help the University improve the quality and hygiene of campus spaces for the entire community. If you would like to receive a summary of the research results once the study is complete, you are free to email one of the researchers listed at the top of this form. Confidentiality and data security
This survey is anonymous. We do not collect names, UTORids, IP addresses, etc. Data will be stored on secure, password-protected University of Toronto servers for a length of 2 years, after which it will be permanently deleted. Only the research team will have access to the raw data. Results will be reported in aggregate (e.g., "80% of respondents felt...") to ensure no individual can be identified. The University of Toronto Research Oversight and Compliance Office – Human Research Ethics Program may have confidential access to the data to ensure that participant protection procedures are being followed. Due to the anonymous nature of this survey, once responses are submitted, they cannot be deleted as no identifiers are used.

Compensation
Upon completion, you will have the option to be redirected to a separate form to enter a draw for a $50 gift card of your choice. The odds of winning are approximately 1 in 75, depending on the number of participants who enter the draw. You will not be reimbursed for any out-of-pocket expenses associated with participating in this survey. Your contact information for the draw will be stored separately and will never be linked to your survey responses. Your contact information for the draw will be destroyed immediately after the winner has been selected and the prize has been awarded.
